Family Case Manager

The Family Case Manager will provide outreach, case management, and coordination of care to individuals and families throughout Miami-Dade and Broward Counties. The ICM will provide client-centered support to help clients and their families develop the skills, knowledge, and resources needed to access benefits, healthcare, mental health services, employment, and other community resources, while working toward improved stability and their individual goals. Responsibilities include conducting intakes and assessments, developing and implementing individualized service plans, providing ongoing case management, referrals, education, monitoring, follow-up, advocacy, and support, and coordinating services with community providers. The ICM will maintain knowledge of available community resources and collaborate with service providers and community partners to connect clients with appropriate support. The position requires strong communication, organization, assessment, and problem-solving skills, as well as the ability to provide empathetic and culturally responsive support while maintaining appropriate professional boundaries. Essential Duties and Responsibilities Responsibilities include the following, and other duties may be assigned: Complete Assessments and Program required documents approved by Department Director Development of Case Management Treatment Service plan Crisis intervention Information and referrals to community resources Arrange for client services, as needed Case coordination and monitoring Assistance with applications for services or benefits as needed Data entry into a web-based software system Maintain confidentiality of all client information and comply with agency policies, procedures, and applicable regulations. Perform other duties as assigned to support program operations and agency objectives. By attending, it helps to set the standard for involvement and demonstrate our commitment to these meaningful traditions. **Please be advised, if selected for this position, you will be required to submit to a level II background screening through the DCF Clearinghouse: Qualifications Bachelor’s degree required; Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or a related field preferred LCSW, LMFT, or LMHC license, or eligibility for licensure, preferred Minimum of two (2) years of relevant experience, preferred Fluency in Spanish strongly preferred Strong written, verbal, interpersonal, and communication skills, with the ability to communicate clearly and document information accurately. Strong proficiency in Microsoft Office, including Word, Excel, and Outlook; accurate data entry and documentation skills are essential.
