Collaborate with onsite NPs/PAs to manage daily patient care and oversee clinical supervision. Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions. Complete timely documentation in ChartEasy and work with facility EHRs (PointClickCare, MatrixCare, AHT). Participate in Theoria’s Chronic Care Management, Transitional Care, RPM, and other quality initiatives.
