Community Health Worker
Health and Welfare Council of Long Island
The Health and Welfare Council of Long Island (HWCLI) is seeking a dedicated Community Health Worker to join our team. This position plays a critical role in bridging the gap between communities and healthcare systems to ensure that the health and welfare needs of Long Island residents are effectively served. The Community Health Worker will work closely with healthcare professionals and community members to provide education and support, connecting individuals with the necessary resources to improve their health and well-being.
