Foster Care Case Planner: Safeguard Kids & Permanency
SCO Family of Services
SCO Family of Services is seeking a Case Planner to collaborate with families to support safety, permanency, and well-being for children and youth in foster care. You will develop and implement service plans, assess progress, and address barriers to achieve goals. You will conduct required visits and documentation, coordinate services with Health Homes, CFTSS, and therapy providers, and participate in trainings to stay current with best practices in child welfare. #J-18808-Ljbffr SCO Family of Services
