Case Planner

This role is a Case Planner in a foster care/child welfare program , responsible for managing cases for children, foster families, and biological families with the goal of achieving permanent placement as quickly as possible. Core responsibilities Assess each child and family’s needs and create/implement treatment and service plans. Provide hands‑on case management: advocacy, problem‑solving, referrals, and life‑skills support. Maintain regular contact with children, foster parents, and biological families, including facilitating visits and supporting relationships. Coordinate with schools, service providers, and community resources to track progress and address barriers. Prepare for and attend court hearings, including testifying when needed. Plan for discharge/permanency by connecting families with community services. Complete extensive documentation (reports, plans, progress notes) and ensure compliance with agency and ACS requirements. Monitor children’s well‑being (education, health, recreation, independent living skills). Participate in meetings, trainings, and supervision. Work environment & expectations Field‑based role with frequent travel, home visits, and flexible hours (including evenings/weekends). Requires collaboration, strong organization, and consistent documentation. Qualifications Bachelor’s degree (BA/BSW) with at least 2 years of human services experience. Child welfare or mental health experience preferred; Spanish bilingual a plus.
