Growing together. * Offering a $10,000 sign-on bonus Job Summary As the Physical Therapist in Home Health, you will complete assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment you will develop a treatment plan, perform interventions to improve and enhance the patient’s well‑being, and evaluate progress. Primary Responsibilities Evaluate the patient’s functional status and physical therapy needs; assist the physician in developing the plan of care. Provide services within the scope of practice as defined by state laws in accordance with the plan of care and in coordination with other health‑care team members. Observe and report the patient’s response to treatment and any changes in condition to the supervising nurse and/or physician. Treat patients to relieve pain, develop and restore function, or maintain maximum functioning using acceptable standards of practice. Instruct the patient, family/caregivers and team members regarding the plan of care, functional limitations, and progress toward goals. Visit patients per the plan of treatment, complete progress notes for each visit, and submit notes per LHC policy. Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines. Report outcomes, goals, and anticipated frequency of care. Required Qualifications Current Physical Therapy licensure in state of practice. Current CPR certification. Proven ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Current driver’s license and vehicle insurance; access to a dependable vehicle or public transportation.
