RN - Home Health

Day shift from 8:00 AM to 5:00 PM, including every other weekend rotation or permanent Tuesday-Saturday/Sunday-Thursday schedules, based on agency needs. Job Description This Pediatric RN position involves providing comprehensive home health care for pediatric patients. You will manage a case load of 20-25 patients with an expectation of 5-6 visits per day. This role requires strong organizational and communication skills, along with the ability to work collaboratively within a team. Responsibilities Conduct home visits to assess and manage pediatric patient care. Perform IV therapy, tube feeding, VAC dressings, and other nursing procedures as required. Utilize EPIC EMR for documentation and patient management. Participate in case management activities, collaborating with interdisciplinary teams. Respond to call duties as required during designated hours. Required Experience / Certifications / Licensure At least 2 years of nursing experience, preferably in home health care. Active RN license in the state of practice. BLS certification is required. Experience with OASIS documentation and proficiency in Epic EMR. Skills in IV therapy, tube feeds, wound care, and more.
