Inpatient Care Navigator/Complex Care Navigator

White Oak Medical Center seeks to hire an experienced Inpatient Care Navigator/Complex Care Navigator RN or SW for our Care Navigation department who will embrace our mission to extend God's care through the ministry of physical, mental, and spiritual healing. As a Care Navigator/Complex Care Navigator RN or SW you will:

• Complete comprehensive psycho-social assessment and review of clinical needs of patients with focus on patient's requirements as they transition to the next level of care.
• Collaborate with physicians, nurses, social workers and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning.
• Identify and navigate patient testing and treatment to reduce barriers to patient discharge and preventing delays in patient care; communicates barriers to leadership for resolution and trending.
• Communicate with Utilization Review staff on any denials, issues or barriers to discharge.
• Identify services and resources available in the community and assists with patient connection to these services.
• Participate in Interdisciplinary Rounds and other patient care conferences.
• Documents assessments and interventions according to departmental standards.
• Participate in process improvement activities.

Qualifications include:

• Registered Nurse (RN): Graduate of an accredited school of nursing with a minimum of an associate of science degree required, bachelor's degree preferred. RN-minimum of 1-3 years clinical experience required. Licensed as Registered Nurse with Maryland Board of Nursing. 3-5 years of inpatient hospital case management experience preferred. Vast experience in discharge planning preferred. Local resources experience preferred.
• Social Worker (SW): Bachelor of Social Work (BSW) or Master of Social Work (MSW). Minimum of 3 years of experience in diverse clinical setting is required. Prior case management or discharge planning experience is preferred. Prior experience with Cerner EMR is preferred. LBSW, LMSW, LCSW. BLS required. 3-5 years of inpatient hospital case management experience preferred. Vast experience in discharge planning preferred. Local resources experience preferred.

Work Schedule:

• Employment Type: Variable Part-time
• Hours per Week: 4 - 8 hours/week
• Typical Daily Schedule: 1 shift a week as needed
• Shift Type: Rotating Day Shift-Weekends and Holidays
• Holiday Requirements: Follow company holiday calendar
• Shift Differentials: Yes

Orientation is expected to be approximately 34 weeks and can be flexible (one week on, one week off).

Pay Range: $77,313.60 - $115,980.80

If the salary range is not displayed or if the position is Per Diem (with a fixed rate), salary discussions will take place during the screening process.

Under the Fair Labor Standards Act (FLSA), this position is classified as: United States of America (Exempt)
