$53 - $56 hourly

Responsibilities:

• Coordinate and manage comprehensive care plans for hospice patients, ensuring their needs are met with compassion and precision.
• Conduct regular assessments to evaluate patient conditions, adjusting care plans as necessary to maintain optimal comfort and quality of life.
• Collaborate with interdisciplinary teams to provide holistic care, fostering a supportive environment for both patients and their families.
• Educate patients and families about the hospice process, empowering them with knowledge and resources to make informed decisions.
• Maintain accurate and timely documentation of patient care activities, ensuring compliance with regulatory standards and organizational policies.
• Serve as a primary point of contact for patients and families, addressing concerns and providing emotional support throughout the hospice journey.
• Facilitate seamless communication between healthcare providers, patients, and families to ensure coordinated and effective care delivery.

Qualifications:

• Experience in hospice or palliative care nursing, with a deep understanding of end-of-life care.
• Ability to develop and manage personalized care plans that reflect each patient's unique needs and preferences.
• Proven track record of coordinating with healthcare providers to ensure seamless, comprehensive care.
• Strong communication skills to provide compassionate guidance and support to patients and their families.
• Ability to conduct regular assessments and adjust care plans based on patient progress.
• Experience in collaborating with interdisciplinary teams to deliver holistic care.
• Proficiency in educating patients and families about hospice services, empowering them with knowledge and confidence.
