About This Role The Complex Care Manager RN will manage complex members, facilitating and advocating for their ongoing self‑sufficiency and independence. This role involves assessment and planning for high‑risk patients, focusing on disease management and maintaining member health. Highlights Location: Charlottesville, VA Schedule: Full Time Pay: $29.00 - $52.00 per hour What You'll Do Engage members/families telephonically and/or face to face to coordinate services, community resources, and treatment needs Deliver a holistic approach to coordinated care based on the member's needs using a person‑centered philosophy Identify early risk factors and conduct ongoing assessments, documenting in an electronic file Create, review, and revise care plans focusing on a holistic approach Function as an advocate for members Collaborate with the member's PCP to deliver and coordinate necessary services Build relationships with members and their families, assisting them with coordination of health choices Ensure cases are documented in a timely manner Coordinate with other state agencies, community resources, and providers Act as the primary point of contact for Members and the Interdisciplinary Care Team (ICT) Requirements Education Current, unrestricted RN licensure in the state of Virginia Experience 2+ years of recent experience in case management, home care, long‑term care, and/or experience in acute or rehab care setting 1+ years of experience directly working with individuals with complex medical or behavioral needs Licenses & Certifications Current RN License in Virginia required Driver's license and access to reliable transportation required Benefits Comprehensive benefits package Incentive and recognition programs Equity stock purchase and 401k contribution About UnitedHealth Group UnitedHealth Group is a leading healthcare provider committed to delivering exceptional patient care.
