Major Responsibilities Assist in developing and implementing individualized treatment plans for residents. Support our CNAs and collaborate closely with our RNs and other team members to deliver exceptional care. Oversee the Falls Program and Pressure Sore Program to ensure resident safety and well‑being. Monitor residents' health and promptly report any changes to the nursing team. Perform additional duties assigned to support the best possible care for our residents. Administer medications and treatments while offering reassurance and support. Listen, comfort, and advocate for the residents – because here, every voice matters. Build strong, trusting relationships with residents, their families, and your fellow team members. Help create a safe, cheerful, and truly home‑like environment for everyone. Minimum Qualifications Active Licensed Practical Nurse (LPN) License in good standing. Experience in Skilled Nursing or Rehabilitation preferred. Strong teamwork and communication skills. Reliability – report to work on time when scheduled. A gentle, respectful approach when communicating with residents. Team spirit and a positive attitude.
