Job Title: Licensed Practical Nurse 
Service Location: Sunbury, PA
Rate: $28-$32 per hour
Immediate Shift Needs: 
Sunbury, Pa:Thursday and Friday 6:30pm to 6:30am, Saturday 8:30am to 8:30pm and Sunday 10:30pm to 6:30am
Selinsgrove, Pa: 8am to 4pm 5 days a week
Northumberland, Pa: 7am to 3pm 7 days a week or Monday through Friday

Responsibilities:

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
