Disability Services Consultant - Bachelor's Required

We are seeking an IRIS Consultant to support individuals in maintaining independence and self-direction through the IRIS program. Responsibilities Include Building trusting relationships with participants Developing IRIS Support and Services Plans Conducting regular contact and visits to ensure participant needs are met Assisting with understanding and maintaining IRIS eligibility Connecting participants with local agencies and resources Accurately documenting all interactions within required systems Possibly conducting annual functional screens Working independently while collaborating with a team Qualifications Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience; or current IRIS Consultant in good standing Bilingual skills are a plus Strong commitment to participant autonomy and respectful service Proficiency with smartphones, tablets, and computer systems Knowledge of social behavior, community resources, and cultural competence Valid driver's license and reliable transportation Key Skills: Empathy, independence, attention to detail, community engagement, adaptability, and strong communication skills. #J-18808-Ljbffr Proforma Health
