About the Role:

• Administer medications, perform treatments, and provide wound care as ordered by physicians.
• Collaborate with other healthcare professionals to ensure seamless care transitions and coordinate patient care.
• Communicate effectively with patients, families, and healthcare teams to ensure patient-centered care.

Responsibilities:

• Conduct initial and ongoing patient assessments, including physical and mental health evaluations.
• Develop and implement individualized care plans, incorporating evidence-based practices and interdisciplinary input.
• Monitor and report patient status, including vital signs, lab results, and treatment outcomes.
• Provide education and support to patients and their families, including discharge planning and follow‑up care.
