Social Services Discharge Planner

As a Social Services Discharge Planner, you will play a vital role in ensuring the smooth transition of our residents back into the community. You will be responsible for planning, organizing, and implementing the overall operation of our Social Services Department, while meeting the emotionally and socially demanding needs of our residents. Responsibilities: Review facility policies and procedures to ensure compliance with state and federal regulations Participate in setting policies concerning resident care and quality of life Develop facility social work policies and participate in quality assurance interdisciplinary team meetings Document progress in meeting the psychosocial needs of residents and work with the interdisciplinary team to promote and protect resident rights Complete social histories and psychosocial assessments for each resident, identifying their social, emotional, and psychological needs Provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility Provide support and education to residents and their families/significant others, referring them to appropriate social service agencies when necessary Coordinate the resident discharge planning process and make referrals for further services Provide clinical interventions to address catastrophic events that occur during a resident's stay Requirements: Bachelor's degree in Social Work or a related field 2+ years of experience in social work or a related field Strong understanding of social work principles, theories, and practices Excellent communication, interpersonal, and organizational skills Ability to work effectively with residents, families, and staff Ability to maintain confidentiality and handle sensitive information Benefits Medical Insurance Dental Insurance Vision Insurance Flexible Spending Account (FSA) Life Insurance Critical Illness Insurance Short-Term Disability Insurance Accident Insurance Hospital Indemnity Insurance 401(k) Retirement Savings Plan On-Demand Pay Option Paid Time Off (PTO) Tuition Reimbursement Program #J-18808-Ljbffr ALIYA Healthcare Consulting
