Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting personalized treatment plans tailored to individual needs
• Implement physical therapy techniques focusing on enhancing mobility, strength, balance, and overall functionality
• Work alongside nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
• Educate patients and their families about treatment goals, discharge plans, and essential home exercises
• Keep detailed records of evaluations, treatments, and patient progress aligning with facility policies and regulatory requirements
• Participate in interdisciplinary care team meetings to ensure seamless coordination of care

Education

• Doctorate in Physical Therapy (DPT) or a similar degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferable
• Specialized certifications (such as neurological or geriatric) are beneficial but not mandatory
