As Case Manager, you will play an integral role in the day-to-day functions of the agency. This position offers a competitive salary and benefit package./ppCase Manager Responsibilities:/ppThe Case Manager is responsible for ensuring that patient care is coordinated and managed appropriately./pulliThe Case Manager is responsible for ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel./liliReceives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs./liliReviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate./liliReviews patients clinical diagnosis, medications, procedures and clinical course./liliAssists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care./liliAttends case conference meetings with organization personnel to facilitate coordination of care./liliConducts quarterly record reviews and communicates findings and recommendations to DPS and appropriate organization personnel./liliComplies with accepted professional standards and principles./liliParticipates in public relation and community activities that promote the Organizations role as an effective member of the health care delivery system./liliPromotes customer service orientation to all organization personnel./liliPerforms other duties and activities as delegated by the DPS./li/ulpInterested applicants are encouraged to apply./ppJob Type: Full-time/pulliHealth insurance/liliPaid time off/li/ulpLicense/Certification:/pulliNew York State RN License (Required)/li/ulpWork Location: In person/p/div
