Primary Care Physician

Job Description: (PACE + no hospital rounds) We are seeking an experienced Primary Care Physician to join a leading PACE (Program of All-Inclusive Care for the Elderly) organization in Stockton, CA . This role focuses on delivering comprehensive, patient-centered care to elderly participants in a collaborative, team-based environment. This is a full-time opportunity offering excellent work-life balance, no hospital rounding, and a low daily patient volume. Responsibilities: Perform comprehensive in-person patient assessments Develop and manage individualized care plans in collaboration with the interdisciplinary team (IDT) Provide primary care and chronic disease management Coordinate discharges (acute, short-term, and long-term care) Conduct home visits as needed Maintain accurate and timely documentation Participate in a rotating on-call schedule (phone only) Requirements: MD or DO (required) Completion of ACGME-accredited residency program Board Certification (preferred) Active California Physician License (unrestricted) DEA registration required Minimum 5–7 years of clinical experience Eligible to treat Medicare and Medi-Cal patients CPR & First Aid certification (or willingness to obtain) Authorized to supervise NP/PA Compensation & Benefits: Salary: $350,000 – $380,000 per year Quarterly bonus potential Full benefits (Medical, Dental, Vision) 401(k) with employer match 17 days PTO + 6 sick days + 12 paid holidays CME reimbursement Malpractice + tail coverage Relocation assistance available Schedule: Monday – Friday (8:00 AM – 5:00 PM) Rotating on-call (few nights/weekends per month – phone only) No hospital rounds Average 8 patients per day Highlights: Work-life balance (no weekends, no hospital rounding) Low patient volume (quality-focused care) Mission-driven elderly care model (PACE) Growth opportunity into Medical Director role To Apply: Email your resume to View email address on click.appcast.io or call View phone number on click.appcast.io
