Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5x8-hour schedule (08:00 - 16:30). Weekends required: 2 weekends every 6 weeks. Call is not required.

Job Description

We are seeking a Case Management RN with at least 2 years of experience. The ideal candidate will work in a dynamic environment, coordinating patient care and navigating complex healthcare systems to ensure optimal patient outcomes.

Responsibilities

• Organize and participate in family meetings.
• Develop and implement treatment plans.
• Conduct needs assessments and determine medical necessity based on evidence-based guidelines.
• Coordinate care for patients, ensuring comprehensive discharge planning.
• Assist with prior authorizations and benefits eligibility.
• Utilize computerized documentation systems effectively, specifically Epic.
• Collaborate with interdisciplinary teams including Physical Therapy, Social Services, and Rapid Response Teams.

Required Experience / Certifications / Licensure

• Must have a valid RN license.
• At least 2 years of case management experience required.
• Experience with Epic charting system is required.
• Experience in LTAC and trauma centers preferred.
• Certification in case management or related field is a plus.
• Ability to float between units based on needs.
