Title
Case Management - Specialty Role Requirements
- Minimum 2 years recent experience
- Experience with Epic charting system
- Current BLS certification
- Familiarity with care coordination, discharge planning, and insurance processes
- Experience with DME/SNF transport arrangements
- Knowledge of InterQual, CMS, NCQA, The Joint Commission, DRG, ICD-10, and HIPAA compliance
- Experience managing a patient ratio of 20 Roles & Responsibilities
- Participate in family meetings and treatment planning
- Independently arrange durable medical equipment (DME), skilled nursing facilities (SNF), and ambulance transport
- Coordinate care with interdisciplinary teams
- Ensure compliance with healthcare regulations and organizational policies Compliance / Notes
- Orientation: 40 hours
- Pre-approved time off: 1 day per contract
- Holiday coverage: Must commit to working 3 of 5 specified holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
- Free parking available
- Must float between facilities within the state as needed
