DDS Assistant Program Manger (Pioneer House)

DDS Assistant Program Manager (Pioneer House) Responsible for coordination and implementation of in-house programming, including assessments, goal planning, evaluation of daily progress, monthly resident reviews, monitoring recreational programs, seeking community resources, and maintaining trip log for van use. Make independent judgments using discretion for the benefit of the individuals. Ensure that individuals receive the highest standard of services and that all services are followed as written in the individual’s plans. Assistant Program Manager must be able to travel, have good writing skills, articulate and be an aggressive advocate. Responsibilities Organize records and assure records are up-to-date. Participate in Interdisciplinary Team meetings/Staff meetings. In conjunction with Program Manager, create agenda for meeting and plan standard items such as committee reports, announcements, and staff concerns. Collaborate with consultants, especially Psychologist on programming. Follow through on recommendations for U.R. committee. Attend U.R. meeting when delegated. Complete and maintain investigation certification as it relates to incidents within the DDS. Ensure all safeguards of life safety situations with individuals are implemented and being followed. Maintain and submit timely and accurate records and reports regarding the health and safety of the individuals and the quality of each program. Submit, update and close all incidents for the program. Maintain compliance with OPWDD and the Justice Center regarding incidents. Make records available for review for IPR & UR. Awareness of and sensitivity to cultural and socioeconomic characteristics of persons and population served. Prepare staff schedules of Program Specialists and handle vacation, personal, and sick leaves as well as cover during any emergency periods of staff shortages. Ensure appropriate staffing pattern as per facility policy. Conduct scheduled supervisor meetings to discuss programmatic and staffing issues. Evaluate individual work performance of all staff through observations and performance counseling in conjunction with Program Manager. Ensure each individual is being provided and/or assisted in skill development in areas such as activities of daily living, domestic living, recreation and socialization as needed. Assist Program Manager in devising and executing relevant agency/program policy in accordance with OPWDD and/or other funding agency guidelines. Conduct frequent rounds on all shifts and submit report to Program Manager to ensure safety and health requirements are observed and quality care and programming are provided. Perform medical assistant responsibilities: track appointments, communicate with CMS, Nurse, doctors and managers, ensure accuracy of medication inventory and order refills, track and follow up on medical appointments, complete medical referral forms and monthly medical book reviews. Coordinate transportation activities to assure maximum use of transportation services for day programming, recreation services, medical and special requirements. Transport individuals to and from programs, community inclusion activities, medical appointments, home visits and all other activities as needed. Complete Staff Information Entry Form when escorting individuals on medical appointments. Ensure timely receipt and ongoing availability of all individuals’ needs (toilet items, clothing, and personal effects). Prepare Day Program progress reports at ITT reviews as assigned. Distribute petty cash funds to staff or collect petty cash receipts and submit them to Program Manager. Conduct monthly fire evacuation drills with all staff members on all shifts. Participate in admission and discharge reviews and make recommendations to Program Manager. Develop monthly recreation schedule in consultation with Program Manager. Implement and monitor in‑house and community recreational activities, providing hand‑on assistance as needed. Other duties as assigned. Direct care as needed. Attendance standards: 1:00 PM – 9:00 PM, Sunday thru Thursday, and at the discretion of the Program Manager. Qualifications High school Diploma with one (1) year experience in Human Services (Social Work, Psychology, Special Ed., etc.) or related field required; Associate’s Degree preferred. One (1) year administrative experience. Two (2) years minimum experience working with individuals diagnosed with intellectual disability (ID). Respect and consideration for the resident being served. AMAP, SCIP‑R and First Aid/CPR Certified. A valid driver’s license is required (must be able to drive a large vehicle which consists of a wheelchair or 15‑seater van).
