Role Summary We are seeking a dedicated and mission-aligned Advanced Practice Provider to join our integrated care team. The Advanced Practice Provider will provide high-quality, age-appropriate, and specialty-specific care to LLCHC outpatients in coordination with a collaborating physician. The APP will practice within their scope of licensure, certification, and education, and will work alongside a multidisciplinary team to deliver holistic care in alignment with LLCHC’s faith-based mission. The ideal candidate is a collaborative leader and clinician who thrives in a community health setting, is committed to serving diverse populations, and is passionate about eliminating health disparities through accessible care. Key Responsibilities Clinical Care Provide comprehensive outpatient primary care, including assessment, diagnosis, treatment, chronic disease management, preventive care, and health education. Maintain clinical documentation in the electronic health record according to LLCHC standards, compliance requirements, and best practices. Ensuretimelyfollow-up on diagnostics, referrals, medication management, and patient inquiries. Participate in a rotating on-call schedule to support patient needs outside regular clinic hours. Quality & Safety Deliver care consistent with evidence-based guidelines, LLCHC protocols, and regulatory standards. Participate in peer review, quality assurance, and performance improvement initiatives. Support population health efforts, care gap closures, and value-based care initiatives. Serve as a collaborating physician for advanced practice providers, including participation in standard-of-care agreements and clinical oversight asappropriate. Contribute to a culture of respect, teamwork, and whole-person care. Leadership & Engagement Model LLCHC’s mission and values in daily practice and interactions. Participate in provider meetings, staff meetings, trainings, and assigned committees to support clinical operations and ongoing improvement. Promote LLCHC in the community and support outreach efforts as needed. Engage in mentoring, coaching, and supporting clinical staff and learners (students/residents), as applicable. Operational Responsibility Use clinic time effectively to support access, continuity, and financial sustainability. Uphold organizational policies, compliance standards, and ethical medical practice. Performadditionalduties as assigned by the Chief Medical Officer or leadership team. Qualifications Current and unrestricted state license to practice as aPsych NP,PA (CAQ-Psych), orPA-C Current or eligible for hospital privileges asrequired. Experience in community health, outpatient care, or underserved populations preferred. EHR experience isrequired. Commitment to LLCHC’s mission and toprovidingcompassionate,equitable, holistic care.
