Care Coordinator RN

We are seeking a Care Coordinator RN responsible for assessing patients' transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure optimal outcomes. Key duties include conducting psychosocial and functional assessments, developing individualized discharge plans, facilitating interdisciplinary care conferences, and managing post-acute referrals. The role involves collaborating with physicians and care teams to monitor care progression, identify and resolve barriers to discharge, and promote efficient resource utilization. Qualifications include a minimum of 1 year nursing experience in an acute care setting , an RN license (single-state or multi-state compact), and BLS certification . Ideal candidates possess excellent communication skills, maturity, self-confidence, and the ability to work effectively under stress in a fast-paced environment. Knowledge of regulations, community resources, and care coordination processes is essential. Additional responsibilities encompass documentation, professional development, and supporting departmental goals, with opportunities to serve as a mentor or preceptor.
