Registered Nurse (RN) - Hospice
Position Overview

As a Hospice RN, you provide end-of-life care focused on comfort, symptom management, and supportive documentation for patients and families.

You work within interdisciplinary teams to honor patient goals and coordinate with community resources for home or facility-based care.

Responsibilities

• Assess symptoms, manage pain and comfort, and provide comprehensive nursing care
• Administer medications and monitor adverse effects; adjust plans in collaboration with the team
• Coordinate with physicians, social workers, chaplains, and home health aides
• Educate families about advance directives, care planning, and bereavement support
• Document care plans and patient status in the EHR; ensure timely communication
• Support discharge planning and transitions to home or hospice facilities
• Respond to clinical changes and provide crisis management as needed

Qualifications

Education:

BSN preferred; ADN acceptable with hospice or palliative care experience

Licensure:

Current, active Registered Nurse license in the United States

Certifications:

• Basic Life Support (BLS)
• Palliative Care certification such as CHPN preferred but not required

Experience or Skills:

Experience in hospice or palliative care; strong communication, assessment, and family-support skills

Experience

• Experience providing end-of-life nursing care in home or facility settings
• Experience coordinating with interdisciplinary teams and community resources
• Experience documenting care plans and patient goals in the EHR

Preferred

• CHPN or equivalent hospice certification
• Experience with care transitions and bereavement support

Work Environment

Hospice teams provide care in patients' homes or hospice facilities; interdisciplinary collaboration is essential

Shifts may include days with on-call coverage; documentation and phone triage are common components
