Nurse Navigator - Full time, Days - Cardiology - Atlantic Health Chilton Medical Center

Responsibilities Exceptional communication, organization and customer service skills. The Nurse Navigator is responsible for enhancing the communication amongst the patient/family, the interdisciplinary team and all interdepartmental needs. Assists in the development of strategies to reduce LOS and re-admissions. Part of the cardiovascular team which monitors LOS and re-admission data. Monitors care process and develops approach to improve care efficiency and patient outcomes. Performs other related duties as assigned.
