Job Description & Requirements RN Case Manager – Home Health StartDate: 9/13/2026 Pay Rate: $1800.00 - $2500.00 POSITION SUMMARY – RN Case Manager – Home Health POSITION DUTIES – Travels to patients’ homes within an assigned service area to conduct assessments, develop and coordinate individualized care plans, provide skilled nursing services, educate patients and caregivers, monitor progress, collaborate with physicians and interdisciplinary teams, manage documentation, and ensure continuity of care. Minimum Required Qualifications – ME/compact RN license BLS 2 years recent home health case management experience Epic and Oasis experience PREFERRED QUALIFICATIONS – Experience with IV therapy, tube feeds, VAC dressings, compression wraps, port access, ostomies, chest tubes, S/p catheters, lad draw, etc. LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – 8a-5p. Every 4th rotating weekend, 3-4 days a month callback. SYSTEMS – Epic
