Responsibilities:

• Promotes Elara Carings philosophy, mission statement and administrative policies to ensure quality of care.
• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care based on thorough assessment and patient participation.
• Conducts therapy within defined standards of care and directs support staff to achieve functional goals.
• Documents evaluations, treatment goals, and plans; maintains required prescriptions and signatures.
• Provides direction and leadership to staff and acts as resource to promote physical therapy in the community.
• Physically demanding environment with full range of motion including handling and transferring patients.

Qualifications:

• Graduate of an American Physical Therapy Association (APTA) accredited program.
• Current, unrestricted PT license for the state of work.
• Minimum one year of experience in a healthcare setting.
• Valid drivers license, dependable vehicle, and current auto insurance.
• Able and willing to travel within the branch/office coverage area up to 50%.
• Ability to lift, push, pull, and support up to 50 pounds, including patient positioning and equipment movement.
