Job Title: Physical Therapist (PT) – Long-Term Care / Skilled Nursing Facility (LTC/SNF)
Location: Seattle, WA
Duration - 13 Weeks
Job OverviewWe are seeking an experienced Physical Therapist (PT) for a Long-Term Care/Skilled Nursing Facility setting. This position offers the opportunity to work with residents requiring rehabilitation services focused on improving mobility, function, safety, and overall quality of life.
Schedule- 8-Hour Day Shifts
- No Weekends
- No On-Call
- Active Physical Therapist License (pending license accepted)
- BCLS Certification required
- Minimum 1 year of Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) experience required
- First-time travelers not accepted
- Reliable transportation required
- Evaluate residents and develop individualized therapy treatment plans.
- Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
- Perform ongoing assessments and adjust treatment plans based on patient progress.
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary teams.
- Educate patients, families, and caregivers regarding rehabilitation goals and home exercise programs.
- Complete timely and accurate clinical documentation.
- Ensure compliance with facility, state, and federal rehabilitation standards.
