Job Summary:

Seeking an experienced Speech Language Pathologist (SLP) for Home Health skilled visits treating adolescent through geriatric patients. The SLP will provide speech therapy services, dysphagia management, patient/family education, and complete documentation for in-home patient care.

Key Responsibilities:

• Perform skilled home health speech therapy visits (6–8 visits per day)
• Provide dysphagia management and aural rehabilitation services
• Conduct patient evaluations and functional maintenance programs
• Educate patients and families regarding treatment plans and safety
• Complete billing, documentation, and OASIS-related charting accurately

Required Skills:

• Dysphagia management experience
• Patient safety and patient/family education
• Functional maintenance program experience
• Evaluation tools experience (BIMS, Depression Screening, Oral Peripheral Exam)
• Documentation and billing experience

Requirements:

• Active State SLP License
• Active BLS Certification
• CCC-SLP required
• Minimum 1 year Home Health experience required
• OASIS training preferred
• Valid driver’s license and reliable vehicle required
• Must be willing to cover Manatee County and South Hillsborough territories
• COVID vaccination required (or approved exemption)
• Flu shot required or signed declination with mandatory mask use

Patient Population:

• Acute and chronic patients
• Adolescent through geriatric patients

• Skilled Home Care patients (Medicare, Medicare Advantage & Commercial Insurance)
