LVN / LPN - LTC/SNF

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

We are seeking a qualified Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a nine-week contract in a long-term care or skilled nursing facility setting. The staff member must adhere to facility protocols and maintain patient care standards throughout the assignment.

Responsibilities:

• Provide direct patient care in accordance with physician orders and facility policies.
• Monitor patient conditions and document vital signs, symptoms, and treatment responses accurately.
• Collaborate with interdisciplinary team members to develop and implement care plans.
• Maintain a safe and clean environment, ensuring infection control procedures are followed.
• Educate patients and families on health management and care procedures.
• Adhere to all facility regulations, including badge and identification requirements.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in long-term care or SNF settings preferred.
• Ability to follow detailed instructions and prioritize tasks effectively.
• Demonstrate strong communication and interpersonal skills.
• Must have completed the COVID-19 vaccination series or possess an approved exemption.
• Ability to wear a photo badge and comply with facility identification policies.
