Compensation: $200-$250 per admission

Benefits: Full Benefits Package for Full-Time employees

Schedule: Full-Time or Part-Time | 1 Backup On-Call Shift Per Week | Every 4th-7th Weekend Rotation

Location: Greater Twin Cities Metro Area | 21-County Service Area (territories assigned based on coverage needs)

What You'll Do

• Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
  
• Perform comprehensive clinical eligibility assessments and initiate hospice services.
  
• Lead conversations about goals of care, hospice philosophy, Medicare benefits, and what families can expect .
  
• Coordinate seamless transitions into hospice with physicians, hospital discharge planners, social workers, intake coordinators, and the interdisciplinary team .
  
• Build trust with patients, caregivers, and referral partners from the very first interaction.
  
• Complete accurate, timely EMR documentation while ensuring regulatory compliance.
  

What You'll Bring

• Active Minnesota Registered Nurse (RN) license.
  
• Experience in hospice, oncology, ICU, palliative care, home health, emergency medicine, or another complex clinical setting.
  
• Outstanding communication, assessment, and relationship-building skills .
  
• Confidence leading sensitive conversations with patients and families.
  
• Ability to work independently in a field-based role .
  
• Valid driver's license and reliable transportation.
