Hospital Social Worker - Discharge & Care Coordination

Ochsner Health is seeking a licensed master level social worker to perform bio-psychosocial assessments, education, and discharge planning within a hospital setting in New Orleans, LA. The role involves coordinating with a multidisciplinary team to address emotional, social, and cultural needs, and may include clinical social work under supervision.
