RN Case Manager (Registered Nurse)

Job Details Location: Bristol Hospice - Virginia Beach, VA 23225 Position Type: Full Time Salary Range: $91,000.00 - $93,000.00 Schedule: Monday - Friday, 8 AM - 5 PM; weekend pickup of Friday 5pm to Monday 8am as a backup on‑call nurse once every 6 weeks Territory: Norfolk, Chesapeake, Va BCH, Portsmouth with occasional patients in Hampton, Newport News, and Yorktown At Bristol Hospice, we believe hospice is not the end—it’s a meaningful journey. We're seeking dedicated RN Case Managers who are passionate about walking beside their patients through every step of this sacred path. Responsibilities Develop and implement individualized care plans based on current and prior medical history Deliver attentive, one‑on‑one support to patients and their families, ensuring comfort, dignity, and understanding Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure Administer physician‑ordered medications and treatments with skill and empathy Educate patients and their families on care techniques and symptom management Track and communicate changes in patient condition, ensuring timely and effective responses Keep detailed and up‑to‑date clinical notes to reflect patient progress and quality of care provided Collaborate with interdisciplinary teams to ensure comprehensive care delivery Contribute to other supportive duties as part of a compassionate, mission‑driven team Perform additional duties as assigned Qualifications A valid Registered Nurse license in good standing in the state where you will work A valid driver’s license and reliable transportation CPR certification (current) Strong observational and communication skills—both written and verbal A flexible, team‑oriented attitude and willingness to support additional duties as needed A heart for hospice care and a commitment to exceptional service Benefits Tuition Reimbursement Paid Time Off & Holidays Comprehensive Health Benefits (Medical, Dental, Vision, Life Insurance, Disability) 401(k) & HSA Mileage Reimbursement Advanced Training Programs Mission‑Driven Culture EEOC Statement Bristol Hospice is an equal‑opportunity employer.
