Registered Nurse Community Crisis Center

Responsibilities Performs general nursing care to patients by following established standards and procedures. Obtains patient consent for care and ensures patient confidentiality. Obtains and documents health history, chief complaint, vital signs, health risk factors, and assesses disease condition. Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations. Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response; refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions. Documents patient assessment and intervention data using established medical record forms and automated systems. Participates in multidisciplinary teams to improve patient care processes and outcomes. Tracks quality assurance data and monitors for acute and chronic care management. Promotes the mission, vision, and values of the organization and abides by service behavior standards. Performs other duties as assigned. Qualifications Minimum qualifications: Graduate of an accredited school of nursing. Current/valid Registered Nurse license from the state licensing board. Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS). One (1) year of related experience in a clinic or acute hospital nursing setting. Bachelor of Science in Nursing (preferred). Additional skills and attributes required: Regular attendance and ability to work on site during scheduled shifts. Knowledge of nursing processes, health care systems, structure, and community resources; understanding of health care technology, equipment, and supplies; familiarity with state law on nursing care, nurse practice guidelines, and clinic policies and procedures. Effective communication skills with staff and patients. Sound judgment and composure with the ability to take appropriate action in questionable or emergency situations. Positive, caring attitude toward patients. Strong work ethic and high level of professionalism. Critical thinking skills within scope of practice, population health management, and improvement of patient outcomes. Physical Demands Frequent interaction with patient care providers, patients, and visitors requiring verbal communication and auditory comprehension, especially during emergencies. Manual dexterity needed to manipulate complex equipment and perform frequent computer use and typing. Ability to lift and transfer patients, bend to retrieve, lift, and carry supplies and equipment of varying weights, up to and including heavy items. Requires walking, ambulating patients, and carrying supplies; includes pushing/pulling gurneys and portable equipment and navigating crowded, busy rooms. May need to stand for extended periods in a stationary position.
