Description

• Conduct assessments of patients within either inpatient or outpatient settings and create tailored treatment plans.
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
• Collaborate seamlessly with nurses, physicians, case managers, and various rehabilitation professionals to achieve optimal patient outcomes.
• Educate patients and their families regarding therapeutic objectives, discharge plans, and at-home exercise routines.
• Meticulously document evaluations, treatments, and patient progress in line with facility and regulatory standards.
• Participate actively in care team discussions and interprofessional conferences to ensure cohesive patient care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.

Certifications

• State licensure as a Physical Therapist
• Preferred CPR certification.
• Specialized certifications (e.g., neurological, geriatric) are beneficial, however, not mandatory.

Skills

• Prior experience in a similar setting is highly desirable.
• Ability to address and treat patients with complex medical and mobility challenges.
