Description

• Assess patients in either inpatient or outpatient environments and develop tailored treatment regimens
• Implement physical therapy interventions aimed at enhancing mobility, strength, stability, and overall function
• Work alongside nurses, physicians, case managers, and other rehabilitative experts to optimize patient outcomes
• Educate patients and their families about therapeutic aims, discharge plans, and at-home exercises
• Maintain thorough documentation of evaluations, treatments, and patient progress per facility and regulatory standards
• Engage in multidisciplinary care team rounds and conferences to ensure coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (e.g., neurological, geriatric) are beneficial but not mandated

Skills

• Prior experience is highly preferred
• Able to treat individuals with complex medical and mobility challenges
• Excellent communication and teamwork skills
• Proficient in digital charting and clinical documentation systems
