Speech Therapist Case Manager

This is a full time position, Monday - Friday. This position will require you to travel to client's homes to provide 1:1 client care that may take 1-3 hours per client.

This position reports to the Buffalo, MN office and provides services in Plymouth and surrounding cities.

Major responsibilities include administering speech and language evaluations, establishing treatment goals, providing speech therapy treatments, maintaining records, guiding clients and families, participating in case conferences, preparing clinical and progress notes, providing education programs, maintaining confidentiality, and performing other related duties.

Physical/environmental demands include light to medium work, ability to drive or use public transportation, and exerting force to move objects.

Qualifications include a graduate degree in speech-language pathology, certificate of clinical competence, two years of experience in a health care setting, strong communication and organizational skills, access to transportation, valid driver's license, proof of negative mantoux or chest x-ray, current CPR certification, U.S. citizenship or valid alien work permit, passing background studies and screenings.

Adara Home Health, Inc. is committed to fair and equitable compensation practices. The base salary range for this position in the U.S. is $90,000 - $105,000 per year.
