Work Environment

This position is based onsite in the patients' homes, allowing for personalized care in a familiar setting. Key Responsibilities

• Conduct thorough evaluations that include reviewing patient histories, performing specific assessments, and documenting results to inform treatment plans.
• Communicate findings and treatment plans to patients and their families, actively involving them in goal setting to encourage engagement and motivation.
• Implement evidence-based treatment protocols tailored to individual patient needs while regularly reviewing and adapting treatment strategies based on progress.
• Collaborate with a wide range of healthcare providers to ensure comprehensive patient care, recognizing and responding to changes in patients' conditions as necessary.
• Manage discharge planning effectively, ensuring continuity of care through meticulous documentation of patient progress, including evaluations and follow-up notes.
• Demonstrate excellent time management skills, adhere to productivity expectations, and keep an organized schedule to maintain efficiency across visits.
• Participate in community outreach and marketing initiatives to promote physical therapy services and enhance patient access to healthcare.
• Engage in professional development opportunities, staying informed about current practices and serving as a clinical instructor for students in the field.
• Uphold the highest standards of customer service, ensuring that each patient interaction contributes to a positive healthcare experience.
• Actively participate in organizational initiatives aimed at removing barriers to care and improving overall service delivery.

Patient Population / Client Focus

You will work with a diverse range of patients, primarily focusing on those in need of home care services due to age, disability, or recovery from surgeries. Qualifications

• Minimum of a Doctorate degree in Physical Therapy (DPT) with current licensure in the State of Florida.
• Current American Heart Association's BLS for Healthcare Providers CPR certification is a requirement for all therapists.
• Strong interpersonal skills, with a commitment to teamwork and collaboration across disciplines.
• Excellent communication skills to effectively engage with patients, families, and healthcare professionals.

Preferred Experience

• A minimum of 2 years of clinical experience as a Physical Therapist is required.
