Concierge Home Care is hiring a Full-Time Home Health Physical Therapist (PT) to serve The Villages, FL 32162 and surrounding areas of Sumter County .

Home health allows Physical Therapists to build meaningful relationships with patients while having greater control over their workday.

As a Home Health PT , you will:

• Evaluate patients in their homes
• Develop individualized physical therapy treatment plans
• Provide therapeutic exercises and functional mobility training
• Address strength, balance, gait, mobility, and safety needs
• Educate patients and caregivers
• Help patients maximize independence and quality of life
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary team
• Supervise and collaborate with Physical Therapist Assistants (PTAs)
• Complete required clinical documentation using streamlined technology

Physical Therapist Qualifications

• Active, unencumbered Florida Physical Therapist (PT) license
• Physical Therapy (DPT or masters-level PT) degree from an accredited program
• Valid driver's license
• Reliable transportation
• Current auto insurance
• Ability to provide physical therapy services independently in patients' homes

 

Home Health Experience Preferred – Not Required

Physical Therapists with experience in home health, home care, outpatient, inpatient rehabilitation, skilled nursing, hospital, orthopedic, neurological, geriatric, or acute care settings are encouraged to apply.
