LPN - Intensive Case Manager (Mental Health) DE

Horizon House, Inc. in Claymont, Delaware is seeking a Licensed Practical Nurse (LPN) to join the Intensive Case Management (ICM) team. You will work on-site, manage recovery plans with participants, and coordinate care with staff and community agencies. Mileage reimbursement is provided and orientation is required. Qualified candidates must hold a Delaware LPN license (or participate in the Nursing Licensure Compact to include DE), with knowledge of mental health and substance abuse systems, #J-18808-Ljbffr Horizon House, Inc.
