Forensic Mental Health Case Manager - Community-Based

Horizon-House in Swarthmore, Delaware County, seeks a qualified individual for a Full-Time role in Non-Residential Direct Care. This position focuses on providing service coordination and case management for clients with support needs due to developmental disabilities and behavioral health issues. Ideal candidates will have a Bachelor's Degree in Human Services, a minimum of 1 year of experience in forensics, and skills to build supportive relationships with clients.
