Connected Healthcare Partners (CHP) is seeking a Telehealth Nurse Practitioner to join the Mobile Acute Care Program. This fully remote role provides virtual evaluation and management for acute and urgent needs, while supporting field clinicians through EHR coordination and care transitions. The NP will conduct telehealth visits via a secure platform, develop treatment plans, and coordinate follow-up care, including transitions from hospital or facility discharge. #J-18808-Ljbffr Complex Care
