In this role, you will support patients and families as they transition from hospital to post-acute settings, overseeing recovery and managing complex medical needs with clinical expertise and empathy.**About the Role**The Physician will provide comprehensive medical evaluation and management for patients in post-acute settings, including skilled nursing and rehabilitation facilities.
