Role Overview Provide in-home comprehensive health assessments for adult and geriatric members, reviewing medical history, medications, social factors, and risks to improve care quality and address care gaps. Role Overview Provide in-home comprehensive health assessments for adult and geriatric members, reviewing medical history, medications, social factors, and risks to improve care quality and address care gaps. Responsibilities Conduct assessments, including medical history, diagnoses, treatments, and health education. Order and administer point-of-care and lab testing. Collaborate with physicians, case managers, social workers, family members, and caregivers. Coordinate with Primary Care Physicians for education and follow-up. Perform services at members' residences, including homes and Skilled Nursing Facilities. Qualifications & Skills Master’s Degree or equivalent with NP licensure and prescriptive authority. Board certification in Adult, Geriatric, Family, or Acute Care specialties. Current BLS, ACLS, or CPR certification. Preferably 1-year NP experience; new grads welcome.
