Position: Nurse Practitioner

Department: Clinical

Reports To: Executive Director

 

GENERAL PURPOSE 

The Nurse Practitioner provides advanced clinical support to enhance hospice medical services. In collaboration with or in the absence of the Hospice Medical Director, the Nurse Practitioner may serve as the medical representative on the Interdisciplinary Group (IDG). This role includes providing medical oversight and direction for hospice care delivered in the home, inpatient settings, respite care, and emergency situations related to the terminal diagnosis. 

 

ESSENTIAL FUNCTIONS  

Medical Oversight & Clinical Care 

- Provides oversight and support of hospice medical services to ensure continuity of care and effective symptom management. 

- Conducts medical assessments, interventions, and consultations across care settings. 

- Documents progression of end-stage disease in the clinical record. 

 

Interdisciplinary Team Collaboration  

- Participates in the development and ongoing review of the Plan of Care (POC). 

- Ensures the POC is established prior to the start of hospice services and updated as needed. 

- Actively participates in IDG meetings to support coordinated, high-quality care. 

 

Clinical Protocols, Quality, & Compliance 

- Assists with clinical protocol implementation and evaluation. 

- Supports decisions regarding continuing care needs. 

- Participates in oversight of inpatient, emergency, and respite services. 

- Contributes to QAPI activities. 

- Assists with resolving clinical or ethical concerns. 

- Educates IDG members, volunteers, affiliated clinicians, and facility staff. 

 

Role Flexibility & Physician Support 

- Serves as primary medical provider when the attending physician is unavailable or relinquishes care. 

- Provides after-hours medical coverage as needed. 

- Acts as liaison to community physicians. 

- Completes additional assignments as directed. 

 

Confidentiality  

- Accesses and protects PHI in accordance with policies and regulations. 

 

QUALIFICATIONS 

Education & Licensure 

- Current Nurse Practitioner license in good standing in applicable state(s). 

 

Skills & Competencies  

- Knowledge of hospice eligibility and certification processes. 

- Commitment to hospice philosophy and palliative care. 

- Ability to collaborate with the interdisciplinary team. 

- Strong communication skills. 

- Must meet all health, background, and vaccination requirements. 

 

ENVIRONMENTAL & WORKING CONDITIONS 

- Work performed in office, home, facility, or community settings. 

- Local travel required. 

- Possible exposure to infectious diseases requiring PPE. 

- Must be able to bend, sit, stand, walk, reach, and lift up to 50 lbs. 

- Participation in on-call rotation required.
