Position Overview Salary: Minimum: USD $58,156.80 per year, Maximum: USD $62,566.40 per year. About Services for the UnderServed (SUS) SUS is a New York‑based nonprofit providing housing and support services to 25,000 individuals and families in NYC and Long Island. For more than 35 years, SUS has advocated for people with intellectual/developmental disabilities, behavioral health challenges, poverty, histories of incarceration, and/or trauma, aiming to right societal imbalances and improve lives. Position Summary The QIDP coordinates all aspects of care for residents, ensuring active treatment in the least restrictive environment, in accordance with OPWDD regulations. Responsibilities Coordinate and conduct annual and semi‑annual reviews of consumer status/progress, assessing risks to consumers’ rights and protection. Ensure completion and receipt of clinical team members’ reports/meetings evaluations; prepare annual CFAs and summaries including a capabilities needs assessment, priority of needs, and a summary of clinical reports, activity schedules, and consumer profiles. Ensure the least restrictive interventions or practices are utilized. Identify practices, interventions, or adaptive equipment that may pose a risk to consumers’ rights or protection and present them to the Human Rights Committee for review or approval. Observe Program Specialist conduct goals active treatment with and provide hands‑on training as needed or prescribed consumers by facilities Plan Of Corrective Action (POCA). Provide staff with ongoing in‑service training. Ensure integration of services between Day Program and the residence and integrate all services into routine daily activities. Complete CSET schedule annually and update as needed; provide and report CSET training to clinicians. Complete Quality Improvement reports as needed and ensure resolution of outstanding issues. Maintain communication with family/correspondent regarding residential programming in conjunction with Social Worker. Qualifications BA Degree in Psychology, Human Services or related field. 2–5 years experience within a Day Habilitation or Residential program. Experience with Day Habilitation plans and Medicaid billing. Detail oriented with exceptional written and oral communication skills.
