Memorial Hermann Health System in Houston seeks a Community Eligibility Specialist to guide patients through NMDOH programs, SNAP, Medicaid and utility assistance. You will screen, document, and educate on eligibility, coordinating with care teams to connect medical and non-medical resources. In-person work at Community Resource Centers, with emphasis on compassionate support and accurate follow-ups. Strong organizational skills, bilingual Spanish a plus, proficiency with Microsoft Office and #J-18808-Ljbffr
