Nurse Practitioner

Position Overview We are seeking a dedicated and compassionate Nurse Practitioner (NP) specializing in Physical Medicine & Rehabilitation (PM&R) to join our growing team in Charlotte, NC. In this full-time role, you will provide high-quality, patient-centered care to geriatric and rehabilitation patients within a skilled nursing facility (SNF). Key Responsibilities Deliver comprehensive PM&R care to geriatric and post-acute patients. Collaborate with interdisciplinary teams including nursing, therapy, and facility administration. Develop and implement individualized treatment and preventative care plans. Build strong relationships with patients, families, and facility staff. Lead initiatives to reduce hospitalizations and improve functional outcomes. Ensure continuity of care and clinical excellence across the facility.
