REGISTERED NURSE - ISUDT (RESOURCE RN) - HIGH DESERT STATE PRISON

Job Title Registered Nurse, CF Location Susanville, CA, Lassen County. High Desert State Prison located approximately 96 miles west of Reno, NV. Job Summary Resource Registered Nurse (Resource RN, CF) under general direction of the Supervising Registered Nurse II/III, Correctional Facility. The Resource RN organizes and provides nursing care for patients participating in Complete Care Model (CCM) initiatives, including Medication Assisted Treatment (MAT), Hepatitis C Treatment (HCV) and other program initiatives. The role serves as the care manager for patients with chronic care management needs and is an adjunct member of the primary care team. Key Responsibilities Organizes and provides necessary nursing care for patients in CCM initiatives, ensuring adherence to policies, procedures, regulations, and program requirements. Serves as care manager for chronic care patients and coordinates interdisciplinary communication to promote continuity of care. Assesses patients for side effects, signs/symptoms of recent drug or alcohol use or withdrawal, mental health, and medical conditions that interfere with treatment. Facilitates recovery, physical health and wellness, medication management groups, and promotes therapeutic relationships. Mentors patient peer development and actively participates in Peer Education Programs. Attends primary care meetings, Population Management sessions, and collaborates with the Primary Care Nurse on care visits, care planning and medication management. Administers injections and provides high-level education to patients. Serves as a tele-presenter for the Telemedicine Addiction Provider and addresses ongoing health care needs of patients. Minimum Qualifications Current, active license to practice as a Registered Nurse in California. Pass the California Registered Nurse examination (Exam Bulletin - Registered Nurse). Terms Limited term 12 months; may be extended up to 24 months or become permanent. Effective July 1, 2025. Salary adjustments per PLP 2025 are applied.
