As a Hospice RN, you will serve as more than just a nurse; you will be a clinical leader, advocate, and a steady guide for patients and their families.

You will take charge of the care plan, manage symptoms, and ensure that each patient receives personalized care that aligns with their goals and preferences. Each visit will allow you to provide not only clinical expertise but also reassurance, clarity, and compassion.

• Lead Patient-Centered Hospice Care:
  • Conduct thorough assessments of patients and their families.
  • Create and manage individualized care plans focused on comfort.
  • Administer skilled nursing care with an emphasis on pain and symptom management.
• Serve as the Care Coordinator:
  • Act as a case manager, coordinating care with the interdisciplinary team.
  • Collaborate with physicians, social workers, chaplains, and aides.
  • Monitor patient conditions and adjust care plans to ensure comfort.
• Support Patients & Families:
  • Provide education, emotional support, and guidance throughout the care journey.
  • Assist families in navigating complex decisions with empathy and clarity.
  • Establish meaningful, trusting relationships during deeply personal moments.
• Drive Quality & Compliance:
  • Maintain accurate and timely documentation.
  • Participate in interdisciplinary team meetings and care planning.
  • Support quality and compliance standards.

• Current RN license (or eligibility for compact licensure).
• Graduation from an accredited nursing program.
• A minimum of one year of clinical nursing experience.
• Strong clinical judgment and a compassionate approach to care.
• Ability to manage a caseload independently.
• Excellent communication skills, especially in sensitive situations.
• Basic proficiency in electronic medical records (EMR).
• Current CPR certification.
• A valid driver’s license and reliable transportation.

• Experience in hospice, palliative care, or home health settings.
• Strong skills in pain and symptom management.
• Experience in case management and interdisciplinary care.
• Ability to quickly establish trust with patients and families.
• Familiarity with Homecare Homebase (HCHB).
